If the tensioner pulley is worn out it could be not "aligned right". Most of the time such a pulley will also make noise when the engine is running.

It is also possible that the tensioner has failed and has allowed the belt to move out of alinement.

Same conclusion: Replace the tensioner and maybe the pulley and maybe the belt too!

Belt Tensioners rarely go bad.  But sometimes bearings can go bad or even the spring inside can break.  If it is not sqeaking and the belt doesn't fly off evertime you start it your ok.  I know what your brother means.  I have a 97 Voyager and the first time I looked at the belt it didn't look like it was lined up right but it was.
