There is no one answer to this, most engine problems can be fixed.

Fluids mixing. One of the worst problems is if the oil and coolant are mixing, This could be a cracked block or head or the head gasket. If this is not caught quickly the engine will go by by.
Really bad noises. Loud clunking sort of noises sometimes means that there is a bad bearing somewhere, difficult to fix, often better to rebuild the engine.
Rough running. If a number of cylinders in your engine are not working they can take the rest of the engine with them. Some times this can ve a simple problem to fix, maybe bad spark plugs or intake gasket leaks.
